Absence Request

If you are planning to take your child away during term time, whether this be for a day, a week or more, you must fill in a leave of absence form for your child/ren.

If you have more than one child attending across the federation you can submit them all on one form, no need to do multiple. 

The form can be found in the foyer of each school at the 'parent post' area on the wall. Alternatively there is a copy attached below which you can either print yourself or submit electronically. Please ensure completed forms are handed in to office staff or sent to the school via email. Email addresses can be found at the bottom of this page.

If you have exceptional circumstance for this absence which you would like to be considered, please supply any supporting evidence at the time of passing over the request.
